What to bring

When you come for your initial consultation with Mr Han, you should bring the following with you:

  • A current referral from your local doctor.
  • All relevant test results you may have, ie X-rays, CT scans or MRI scans, nerve conduction studies or pathology results.
  • Completed patient registration form. The registration form may be downloaded from this website or we can arrange for one to be posted to you. Click here to download our Patient Registration Form

Consultation Times

The main rooms in Fitzroy are open from 9.00am to 5.00pm, Monday to Friday.

Consulting times/days:

Coburg – One Saturday per fortnight, 9.00am to 1.00pm
Fitzroy – Tuesday, Wednesday and Friday
Warragul – One Monday per month, 8.30am to 5.00pm
Warrnambool – One Monday per month, 8.30am to 5.00pm
Werribee – One Wednesday per month, 8.30am to 12noon

Consultation Fees

  • Initial consultation fee is $350, and $250 for subsequent consultations. Medicare reimburses $127.05 for the initial consultation and $84.15 for subsequent consultations.
  • Consultation fees must be paid in full on the day of service and we can send your Medicare claim to Medicare electronically on the day, if you wish. This can only be done in the main rooms. If your appointment is at one of the other locations, please contact us after your visit and we will send it through for you.
  • A discounted fee may be available to pensioners and healthcare card holders.
  • DVA Gold Card holders will have their accounts forwarded directly to DVA.
  • Please note Mr Han does not see WorkCover or TAC related patients
